1. Liability & Assumption of Risk

I hereby acknowledge that participation in physical exercise, fitness classes, personal training, and related activities involves inherent risks of physical injury, illness, or property damage. I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in these programs.

2. Medical Clearance

I certify that I am in good physical condition and have no medical impairment, disease, or other illness that would prevent my safe participation in these activities. I understand it is my responsibility to consult with a physician prior to commencing any fitness or training program.

3. Release of Liability & Waiver of Right to Sue

In consideration of being allowed to participate in these training programs, I hereby knowingly, voluntarily, and expressly waive, release, and discharge the training facility, its owners, coaches, employees, and independent contractors from any and all liability, claims, demands, actions, or rights of action for any personal injury, death, or property damage arising out of my participation, even if caused by the negligence of the facility or its staff. I agree that neither I, nor my heirs, assigns, or legal representatives, will sue or make a claim against the facility or its personnel for any such loss or damage.

4. Media Release & Video Recording Agreement

I understand that classes and training sessions may be filmed, photographed, or recorded by the facility for instructional, security, or marketing purposes. I hereby grant the facility the absolute and irrevocable right to use, edit, publish, reproduce, and distribute my name, voice, likeness, and image in any video or photographic format across any platform (including social media, website, and promotional materials) without compensation, review, or further approval.

5. Governing Law

I agree that this agreement shall be governed by and construed in accordance with local laws, and that if any portion of this waiver is held invalid, the remainder shall continue in full force and effect.

By signing below, I acknowledge that I have read this document in its entirety, fully understand its terms, and understand that I am giving up substantial legal rights, including my right to sue.
